• Resource Lists are available on a variety of topics (i.e., nursing homes, senior housing, retirement/assisted living, home care services, adult day care/respite, senior centers, dental resources, utility discounts, employment programs, and transportation options)

Assistance with Accessing Services...

• In-depth assessment by phone to determine client needs and, if appropriate, refer to other community services or programs.
• Phone consultation for family members (or other involved persons).
• Assistance by phone to help older adults receive appropriate benefits and services.
• Pre-screening for the Snohomish County Respite Program (18+).
• A contact point for referrals to DSHS - Home and Community Services for people who may need state-funded in-home services.
• Eligibility screening for the Snohomish County Case Management Program (60+).
