• In-depth assessment by phone to determine client needs and, if appropriate, refer to other community services or programs.
• Phone consultation for family members (or other involved persons).
• Assistance by phone to help older adults receive appropriate benefits and services.
• A contact point for referrals to DSHS - Home and Community Services for people who may need state-funded in-home services.
• Eligibility screening for the Snohomish County Case Management Program (60+).
